- SMS Athletic Code of Conduct (required every year)

- HMS Athletic Participation Permit AAPP (required every year)
- Health Insurance: valid proof that the athlete is insured is necessary. Complete insurance information on the AAPP form above. School insurance is available. Parents are responsible for reporting medical or insurance changes to the school.
